Cause for Concern Action Form

Please fill out the below CFC Action form should you have any concerns relating to safeguarding of both children, vulnerable adults, chaperones, crew and adult members.

This form will be automatically sent to the Safeguarding Officer of Hyde Festival Community Association who may contact you should any further information be required. The officer may also use this form and information provided and will report and escalate the CFC form should the need arise.
